Privacy Policy


Your privacy matters. I am committed to safeguarding your personal information and personal health

information in compliance with Ontario’s Personal Health Information Protection Act (PHIPA) and the

Professional Practice Standards of the College of Registered Psychotherapists of Ontario (CRPO). This

policy outlines how I collect, use, disclose, store, and protect your information.

Information Collected

I collect only information necessary to provide safe, effective psychotherapy services. This includes

identifying information, contact details, health and mental-health history, assessment information,

session notes, billing information, and communications.

Purpose of Collection

Your information is used for assessment, treatment, care coordination (with consent), booking, billing,

regulatory compliance, and safety-related obligations.

Use and Disclosure

I will not disclose your information without your consent except when required by law, including risks of

serious harm, child/vulnerable person protection concerns, or court orders. With your consent,

information may be shared with other health providers or insurers.

Your Rights

You have the right to access your clinical record, request corrections, withdraw consent (where

possible), and ask how your information has been used or disclosed.

Storage and Security

Records are stored securely using encrypted digital systems and locked physical storage. I use strong

passwords, encrypted devices, and secure platforms to protect information.

Retention and Disposal

Records are retained for a minimum of 10 years after the last contact, or 10 years after a minor turns

18. Records are securely destroyed when no longer required.

Electronic Communication

Electronic communication may not be fully secure. Please use discretion when sharing sensitive

information electronically.

Privacy Breaches

If a breach occurs, you will be notified promptly and corrective steps will be taken. If required, I will

report the breach to the Information and Privacy Commissioner of Ontario.
